To exercise the right to maintenance assistance, allowance for assistance and care of another person or personal disability allowance (1200, 500 or 300 kn depending on the decision), it is necessary to collect and submit the following documents as evidence:
- Birth certificate for all members of the joint household (original and copy)
- Birth certificates
- Copy of ID card for all adult members of the joint household
- Copy of marriage certificate if you are married if not there is no connection as it is not necessary that you are all at the same address it is important that the person submitting the application (mother or father) has the same address as the child.
- Income certificate for all members of the household for the previous three months (if you are a private accountant the accountant will calculate the last 6 months for you.)
- Income certificate from the Tax Administration
- If you are a student – confirmation from the university that you are studying if you are unemployed then a certificate from the bureau.
- Recent medical documentation, (but submit everything) medical findings, discharge letter (copies), (neurologist, physiatrist, if the child is older than three I think then a speech therapist and psychologist are needed)
When you receive this decision and if it is positive, either 1200, 500 or 300 kn, you are automatically entitled to an increased child benefit of 832.00 kn.
Then you report to the HZMO for child benefits and bring the decision you received at the center along with your birth certificates, and then income does not matter.
